Testosterone is a vital hormone responsible for various functions in the male body, including maintaining muscle mass, bone density, mood, energy levels, and sexual health. Some men experience low testosterone, a condition known as hypogonadism, which can cause symptoms like fatigue, reduced libido, erectile dysfunction, low mood, and muscle weakness.

Testosterone Replacement Therapy (TRT) can help restore hormone levels to a healthy range, alleviating symptoms and improving quality of life. One common form of TRT is the use of transdermal gels, such as Testogel & Tostran.

What is Testogel?

Testogel is a clear gel containing testosterone, designed for external application on the skin. It allows testosterone to be absorbed through the skin into the bloodstream, mimicking the body’s natural hormone release.

How to Use Testogel

  1. Dosage: Your doctor will determine your dosage. The typical starting dose is 50 mg (one sachet or 2 pumps) applied once daily.

  2. Application Site: Apply Testogel to clean, dry, and unbroken skin. Recommended areas include the shoulders, upper arms, or abdomen.

  3. Application Tips:

    • Do not apply to the genitals or areas with cuts or irritation.

    • Allow the gel to dry completely before dressing.

    • Wash your hands thoroughly after application to prevent transfer.

  4. Avoid Skin Contact with Others: Ensure the gel does not transfer to others, particularly women and children, by avoiding skin-to-skin contact until the area is covered or washed.

Blood Tests Required

To ensure safe and effective use of Testogel, regular blood tests are essential:

  1. Baseline Testing:

    • Total testosterone levels (morning sample, before 10 am).

    • Full blood count.

    • Prostate-specific antigen (PSA).

    • Liver function tests.

  2. Follow-Up Testing:

    • Testosterone levels: to confirm absorption and ensure levels are within the desired range.

    • Haematocrit: high levels may indicate excessive red blood cell production.

    • PSA: to monitor prostate health.

    • Liver function tests.

Typically, blood tests are conducted 3–6 months after starting TRT and then annually.

Benefits of Testosterone Replacement Therapy

  • Improved Energy: Increased stamina and reduced fatigue.

  • Enhanced Mood: Alleviation of low mood and improved mental well-being.

  • Better Libido and Sexual Function: Increased interest in sex and improved erectile function.

  • Muscle and Bone Health: Increased muscle mass, strength, and bone density.

  • Cognitive Benefits: Enhanced focus, memory, and mental clarity.

Possible Side Effects

While TRT can provide significant benefits, some men may experience side effects, including:

  • Skin Reactions: Irritation or rash at the application site.

  • Increased Red Blood Cells: This can thicken the blood, increasing the risk of blood clots. Regular monitoring of haematocrit is essential.

  • Prostate Health: Possible stimulation of prostate tissue. Men with a history of prostate cancer should discuss risks with their doctor.

  • Hormonal Imbalances: Too much testosterone can lead to acne, mood swings, or breast tenderness.

  • Fluid Retention: Swelling of the ankles or feet.

Important Considerations

  • Contraindications: Testogel is not suitable for men with untreated prostate or breast cancer, severe heart or liver conditions, or untreated sleep apnoea.

  • Lifestyle Factors: Optimise your diet, exercise, and sleep alongside TRT for maximum benefit.

  • Regular Monitoring: Work closely with your healthcare provider to adjust dosage and monitor for any complications.

Testosterone replacement therapy, when used appropriately and under medical supervision, can significantly improve symptoms of low testosterone, restoring vitality and well-being. If you suspect low testosterone, consult your healthcare provider to discuss your symptoms and explore testing options.
